Pharmacological properties cen-pen-suspension

Cen-a-pen suspension is composed of procaine benzylpenicillin which is a beta-lactam antibiotic that is included within the natural penicillins of group G, is exclusively parenteral and with a reduced spectrum.

It also has an action that is fundamentally bactericidal against most Gram + bacteria and against a limited number of Gram- bacteria (usually in the urinary environment), as well as against certain spirochetes and actinomycetes, including in its spectrum of action.

It also contains dihydroestreptomycin that works as a bactericidal aminoglycoside antibiotic , active against Gram – and some Gram + bacteria, including in its spectrum of action.

The association of both compounds generates a bactericidal effect on Gram + and Gram- bacteria.

Mechanism of action: Procyan benzylpenicillin acts as a blocker of the biosynthesis of the bacterial wall. It is fixed by the covalent junction after the opening of the beta-lactam nucleus, on certain PBP enzymatic proteins. It should be noted that penicillin is active only on bacteria in the multiplication phase.

In the case of dihydroestreptomycin, it acts on the 30S unit of the ribosomes which prevents the initiation phase above all, disrupting the ordering of the messenger RNA and causing an incorrect reading of the genetic code by the transfer RNA. It also disturbs the permeability of the bacterial membrane.

Thanks to the association of both compounds, synergism is achieved due to a first effect of penicillin on the bacterial cell wall, allowing dihydroesteptomycin to penetrate more easily into the cell, thereby increasing the levels of efficacy of both compounds separately.

Some microorganisms mentioned above may become resistant to the product through the production of beta-lactamases, which break the beta-lactam ring or penicillins, causing them to be inactivated. In addition, there are cross-resistance with other aminoglycosides such as streptomycin, neomycin, gentamicin and kanamycin.
